Support Coordination

Level 2

The aim of support coordination is to strengthen a participant’s ability to design, and then build their supports with an emphasis on linking to the broader systems of support.

Put simply, support coordinators will walk alongside you on your journey. Some areas that we support you with are:

- Understanding the NDIS jargon so that you can understand your NDIS plan

- The funding allocations and how it relates to your goals

- Link you to service providers

- Build your capacity and support network

- Report writing

Psychosocial Recovery Coaching

Psychosocial Recovery Coaching provides similar supports as a support coordinator but focuses on people living with a psychosocial disability. The NDIS recognises the episodic nature of mental illness and have designed the Psychosocial Recovery Coaching role to be able to maintain engagement through periods of increased support needs.

We will also help you with:

- Developing a mental health action plan so that your support network knows what works best for you when you are not 100%

- Building your capacity to identify when you may require an increase in supports

- Learning strategies that work well for you to improve your mental and physical wellbeing

My name is Chloe Hutchesson

I am an experienced Support Coordinator, Psychosocial Recovery Coach and Mentor. I have an honours degree in Psychology and a passion for empowering people. I pride myself on being friendly, patient and of a caring nature. I started Little Blossom Support so that I can provide quality service to people living with a disability. Whether this is your first NDIS plan, or your 4th, I am eager to help you understand and get the most out of it.
